Additional Information:

  1. In order to process the application, you have to submit the additional documents along with the application.
  2. Completed and signed application
  3. Copy of Articles of Incorporation, Partnership Certificate–if you are a partnership, Assumed Name Certificate–if you are a proprietorship (DBA) and Federal EIN.
  4. Copy of each owner’s, shareholder’s, and principal’s Driver’s License.
  5. Most recent profit-and-loss statement and balance sheet.
  6. Accounts receivable aging or a list of all outstanding invoices.
  7. Master customer list with names, addresses, and telephone numbers.
  8. Copy of a sample invoice with backup documentation (UPS number, purchase order, shipping order, signed delivery sheet).
